An Overview of Infrapatellar Bursitis


There are 2 infrapatellar bursa in each knee, the 'deep' infrapatellar bursa and the 'superficial' infrapatellar bursa:

  • The deep infrapatellar bursa lies below the kneecap between the patella tendon and the upper front surface of the tibia (shin bone).
  • The superficial infrapatellar bursa acts as a cushion between the patellar tendon and the skin.

Due to the location of the bursa, pain will make kneeling difficult. This type of bursitis is also called Clergyman's Knee because it was a common condition among clergymen kneeling for prayer.

Symptoms of Infrapatellar Bursitis

Pain around and below the knee cap is commonly caused by prepatellar or infrapatellar bursitis.

You might be suffering from infrapatellar bursitis if you are experiencing one or more of the following symptoms:

  • Knee Pain - you may experience sharp pain and tenderness just below the kneecap on the inside part of the knee.
  • Knee bursitis pain becomes worse during exercise or when you are climbing stairs, but can also increase by simply bending and straightening your knee. Knee pain caused by bursitis usually starts gradually and develops over a few days or even months.
  • Difficulties Sleeping - Patients suffering from knee bursitis may also have interrupted sleep patterns. Simply bending your knee or rolling over during sleep can cause pressure on the inflamed bursa, increasing your pain. The pain can range from mild to very sharp depending on the amount of swelling and inflammation in the bursa. Many knee bursitis sufferers recommend placing a pillow between your knees/thighs while you sleep to reduce pain throughout the night.
  • Swelling, Tenderness, Warmth, and Redness - Swelling is usually evident with infrapatellar bursitis and often appears as an abscess above or below the kneecap. This swelling may appear immediately following trauma, over a couple of hours, or up to 7-10 days after the event that caused the irritation. Warmth and visible redness will increase in the area as inflammation becomes more severe in the bursa and along the tendons.
  • Limited Range of Motion and Weakness - Pain will most likely limit your knee movements as you stop performing motions that make the pain worse. If bursitis is left untreated the inflammation can increase, causing more pain, limiting movement even more. Eventually, weakness in the leg muscles and and tendons around the knee can set in. As you lose strength in your quadriceps, your legs may begin to give out during daily activities.

If any of those statements are true for you or you're suffering from on-going pain on the inside of your knee then you might have an injury called Infrapatellar knee bursitis.

Infrapatellar bursitis occurs when one or both of the bursa sacs inside the knee become irritated and inflamed resulting in swelling and knee pain. As mentioned earlier, bursitis is quite painful - this is due to the bursa sac becoming irritated and inflamed, affecting the bursa and the soft tissue around it. A bursa is a sac of synovial fluid, rich in protein and collagen. These fluid-filled sacs form in areas where 2 surfaces in your body, most often where a bone and tendon or a bone and muscle, rub together during movement. When pressure or friction is too high, excess fluid can build up in the bursa sac and/or the the lining of the sac can thicken, causing inflammation. When a bursa becomes inflamed, moving your knee becomes painful and difficult. Any actions that put pressure on the inflamed bursa can increase irritation and cause further inflammation and pain.

Infrapatellar knee bursitis is a very real injury affecting the bursa sac in your knee. This form of knee bursitis can happen to anyone as this is often caused by acute trauma to the knee. Much like prepatellar bursitis, infrapatellar bursitis can also occur due to repetitive pressure applied to the knee as often occurs when crawling, scrubbing floors or excessive kneeling. As an example, infrapatellar bursitis is common in people whose nature of work requires kneeling for long periods of time as is common in floor cleaners, floor installers and roofers.

If you are experiencing a significant amount of swelling or pain, it is possible that you have a broken or chipped patella. This is why it is always important to see your physician and get a proper diagnosis. In this case, your doctor may recommend an x-ray to examine this further.

Once a bursa is injured, fluid and/or blood is allowed to enter into the bursa and inflammation will increase over time. Once this happens, the bursa sac walls may thicken and form scar tissue or calcium may collect in the bursa. If patellar bursitis was incurred due to the nature of your work, you will have very little chance of getting rid of this condition unless you change or stop the work that caused it until it is completely healed.

Causes Of Infrapatellar Bursitis

Inflammation of the infrapatellar bursae or the surrounding tendons (quadriceps tendon and patellar tendon) usually develops due to overuse, trauma, or degeneration of protective tissue in the knee. Specific causes include:

Acute Trauma - A direct blow to the knee, at the top of the tibia, can cause injury to the infrapatellar bursa during athletics or when falling on the knee. In such cases, blood may leak into the bursa - causing irritation, inflammation, swelling and pain.

A blow to the knee can cause damage to the infrapatellar bursa.

Excess Stress - Without exercise or activity, muscles can weaken and your knee no longer has the muscle support it needs to move correctly. This can cause excess load on the knees and hips - or your weight may be distributed unevenly within your knee joint. Uneven distribution of weight will put extra stress on some areas compared to others leading to abnormal wear and tear on the stressed areas.

In addition, if you are overweight, the knee joints have to work harder to carry a greater load causing more stress on the tendons, ligaments, muscles, bones, and bursae in the knees. The excess stress can lead to irritation and inflammation of any of the soft tissue over time, including the infrapatellar bursa.

Overuse (Chronic) - Excessive friction or rubbing on the infrapatellar bursa can aggravate it over time. If during work or exercise, your routine requires frequent climbing, running, jumping, or hiking you may be at higher risk of infrapatellar bursitis due to the frequent knee bending. This risk increases if there is anything that may cause abnormal wear and tear within the joint.

Kneeling for extended periods of time can cause bursitis in the knee.

Pressure on the Bursa - Work and frequent activities that place repetitive pressure on the infrapatellar bursa can cause infrapatellar bursitis. Although the bursae in your knee function as cushions to protect the tendons and other soft tissue, frequent kneeling during your work or activities often creates more stress than they can handle. The excess pressure causes the bursa to become irritated and inflamed, leading to swelling and thickening of the bursa lining. When kneeling on hard surfaces, knee pads are recommended to treat or prevent infrapatellar bursitis.

Infection (Septic Bursitis) - The closer the bursa is to the surface of the skin, the more likely the chance of infection from specific bacteria that are commonly found on the surface of the skin. This infection is known as septic bursitis and common bacteria that create this infection are known as Staphylococcus Epidermis (or Staphylococcus Aureus). This bacteria can affect the bursa if the knee is cut or scraped, allowing it to get under the skin.

How Do I Diagnose Infrapatellar Bursitis?

When you are experiencing knee pain, the best way to diagnose this condition is with a quick visit to the your doctor for a physical examination of your knee. Getting a proper diagnosis is important so you can treat your condition correctly. An untreated chronic inflammation of knee bursitis condition can lead to calcification of the bursa. Calcification of the bursa will mean that it will become even more painful than before.

To begin with, your doctor will gather a medical history about you and your current condition and symptoms. He/she will ask about the amount pain you are having with your knee. How long you have had your symptoms and if you are experiencing a range of motion loss with the leg and knee. Details about what caused the pain in the knee, when it started, and whether or not you have ever had treatments for this or a similar condition in the past, are very helpful in assessing your injury.

Range of motion movements / tests will be done to see how much movement has been lost in your knee. Your knee might hurt from the doctor poking and prodding at it, but by doing this the doctor will better understand the source of your pain, the level of tenderness in and around your knee and how well your knee can move with this condition.

Your doctor may also consider any previous knee injuries or joint stiffness that you may have had in the past. This will help the doctor to determine if you have a more complex injury in your knee and rule out any other foot injuries that may be present.

If a soft tissue injury is suspected, an MRI may be undertaken to view where and how extensive the damage is. An x-ray may be recommended to rule out other possible causes of your symptoms. Possible causes of pain that an x-ray can identify include instability, abnormal bone shapes (bone spurs or bone cysts), wear and tear on the joints, avulsion fractures (when a fragment of bone tears away from the main bone along with the muscle, tendon, or ligament as a result of trauma), dislocations, arthritis, calcium deposits in the bursa or joint, and/or other problems.

As the bursae in the knee are close to the surface of the skin, they are more susceptible to septic bursitis (caused by a cut or scrape) than most other bursae. Your doctor may aspirate (use a needle) to remove fluid from the bursa to check for possible staphylococcus epidermis (or staphylococcus aureus) bacterial infection. Your doctor will be able to determine whether there is an infection or not by drawing a small sample of the bursa fluid with a needle. If it is determined that your bursitis is septic, you will be prescribed antibiotics to get rid of the infection.

Infrapatellar Bursitis Home Remedies - What You Need To Know

If you suspect you may have bursitis, the first thing to do is consult your physician; only your physician can give you a proper diagnosis and from this, determine a course of proper treatment. When it comes to bursitis, this is especially important, as septic bursitis (an infected bursa), can be a life threatening problem and must be dealt with immediately.

The most important factor in healing infrapatellar bursitis is resting your knee. This can be difficult when you have to carry on with daily activities, but resting whenever you can is recommended. During your recovery you will probably have to modify or avoid the activities that stress your bursa until your pain and inflammation settle.

Most cases of infrapatellar bursitis will heal with simple home conservative treatments and surgery is often not needed

It's generally understood by doctors and surgeons that surgery will introduce more scar tissue into the tendons and ligaments in your knee. This added scar tissue will be problematic, requiring visits to a physical therapist (PT) and conservative treatment options post-surgery. When it comes to infrapatellar bursitis there's only a few surgical options for treatment - removal of the bursa sac, scraping of the bone(s) to allow more room for the new bursa to grow back. This is why surgery is only performed as a last resort for chronic knee bursitis that won't heal with conservative treatment methods.

Relieving the pain of your sore knee and the symptoms of bursitis initially focuses on keeping the pressure off the bursa. If your infrapatellar bursitis was caused due to the nature of your work, you really need to stop that type of work until the bursitis is gone. For non-infectious bursitis, the preliminary treatment will almost always start with conservative treatment options.

Conservative treatment options include:

  • Rest - This is important for initial recovery; rest and elevation will help reduce pain, swelling and inflammation in the early stages of injury. This can be difficult when you have to carry on with daily activities, but resting and elevating your knee whenever you can is recommended. During your recovery you will probably have to modify or avoid the activities that put stress on your knee until your pain and inflammation settle. However, too much rest can also be harmful, as knee and ankle immobility can actually cause stiffening in the joints. This is why rest should be used when reducing initial pain and swelling, but should not be considered for more long-term conservative treatment.
  • Increase blood flow to your knee
  • Avoid Activities that Caused Your Infrapatellar Bursitis - For infrapatellar bursitis, do not sit on your knees as the ground will likely pressure the bursa. While resting your knee it's also important to avoid all activities that may have caused or exacerbated the problem (especially any strenuous or repetitive movement). Continuing on with activities will likely increase the severity of your injury, turning a mild to moderate case of tissue damage into a downward spiral of worsening damage that may eventually severely impact your life. Also, trying to 'work around' your injury will eventually give rise to over-compensation injuries in other areas of your body (like your opposite foot, or hip & knee pain due to your limping gait).
  • Use a Cold Compress or Ice Pack - Cold is very effective at reducing pain and inflammation - use at the onset of the injury and during flareups. Immediate pain relief and reduced inflammation can also relieve some of the pressure that's being placed on your bursa although cold temporarily shortens connective tissue (tendons, muscle, fascia) causing more friction on the bursa sac. Generally, use cold when there is swelling but stop once swelling is down. In cases of chronic (long term) bursitis, there is typically little to no inflammation.

  • Rehabilitative Stretching under supervision of a PT or doctor. The intent of this is to provide you with increased range of motion, pain relief and strengthening of the surrounding tissue of the joint. Doctors or surgeons typically won't perform a surgery until they feel that their patient has put effort into treating their injury with conservative treatment methods. This may include up to 4 to 6 months of visits to a PT clinic. If you haven't experienced any improvement in your condition during that time then surgery may be considered.
  • Stretching - Stretching your joint in PT and at home will help you to regain your range of motion much faster than not stretching at all. Stretching in many ways is key maintaining good Range of Motion (ROM) in your joint, and stretching can be made much easier with use of a TShellz Wrap® before to warm up soft tissue, and a Cold Compress or Ice Pack treatment after to prevent any return of swelling and inflammation.

Conservative Treatment General Guidelines


Stage 1: If the bursitis was incurred recently or if there is still significant inflammation and pain - use a good quality cold pack and Arnica Pain Relief Cream. If the injury has been going on for awhile and there is no swelling, skip to Stage 2 below.

Recommendation 1: If you are experiencing pain and swelling in the very early stages of the bursitis (first 48 to 72 hours), the first step in a conservative treatment protocol would be to focus on reducing the discomfort by applying a quality cold pack to the injured knee along with a high quality pain relief cream such as our Arnica Infusion Cream.

Note: If you are suffering from a bursitis and/or tendinitis injury that has been going on for awhile, you may not need much cold treatment as inflammation may be minimal. Anytime you are experiencing significant pain, you will most likely find our Arnica Infusion Pain Relief Cream to be quite helpful.

Not only will you use your cold pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...

  • after any activity that stresses the knee in a moderate way. It is wise to quickly apply cold to not only reduce the pain, but to also minimize tissue damage that may have occurred during the prior event.
  • If you have a job that requires you to place some demands on your knee (ie. climbing stairs, kneeling, driving). If that is the case, use cold immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
  • If you are in a post-operative situation where cold compression would be utilized heavily for at least the first few weeks following the procedure. Similarly, if you are scheduled for upcoming knee surgery, then your doctor or surgeon might recommend undertaking cold before the surgery.

Having a Cold Compress or Ice Pack available at home to use if and when the need arises is helpful. Many of you already do so.

However, too many people over-focus on cold while ignoring the most critical aspect to healing - nourishing the injured tissues with a healthy supply of blood flow needed to repair and rebuild the injured tissues.

Stage 2: Focus on Increasing Blood Flow To Injured Soft Tissue - As This is The ONLY Way It Will Heal For the Long-Term

Generally it is accepted that heat on soft tissue (muscles, tendons, ligaments) provides the following desirable therapeutic effects:

  • Tissues heated to 45 degrees Celsius and then stretched exhibit a nonelastic residual elongation of about 0.5 to 0.9 percent that persists after the stretch is removed. This does not occur in these same tissues when stretched at normal tissue temperatures.
    Stretching of tissue in the presence of heat would be especially valuable in working with ligaments, joint capsules, tendons, fasciae ... that have become scarred, thickened, or contracted. Such stretching at 45 degrees Celsius caused much less weakening in stretched tissues for a given elongation than a similar elongation produced at normal tissue temperatures.
    Experiments cited clearly showed low-force stretching could produce significant residual elongation when heat is applied together with stretching or range-of-motion exercises. This is safer than stretching tissues at normal tissue temperatures.
  • Muscle spasms have long been observed to be reduced through the use of heat, be they secondary to underlying skeletal, joint, or neuropathological conditions.
  • Heating muscles produces an increased blood flow level similar to that seen during exercise. However, moving when you're injured is hard. Since moving while injured can be painful most people think it's better to stop moving, rest their foot and hope that their injury will heal all on its' own. Even though rest is important to recovery, too much rest during the recovery process will increase the amount of scar tissue in the joint.
