Frequent wrist movement can lead to wrist bursitis.

The wrist is made up of eight small bones several tendons and ligaments, and 2 bursae; the ulnar bursa and the radial bursa. The ulnar bursa covers the tendons of the index, middle and ring fingers. The radial bursa covers the thumb tendon and extends to the wrist crease. Infection in the radial bursa and ulna bursa is known as a "horseshoe abscess".

The wrist is constantly in motion throughout the day. Simple everyday activities such as getting dressed, tying shoes, opening a door and preparing dinner are difficult to do when you have a painful wrist. The extensive use of the wrist puts it at a considerable risk for injury; athletes are at an even higher risk: 9% of all athletic injuries involve the wrist.

Your bursae* play an important role in leading a healthy, active life. When the bursae are not irritated and working properly, your joints move smoothly and painlessly. However, when a bursa becomes swollen and inflamed, the condition is known as bursitis. When pressure or friction is too great, excess fluid can build up in the bursa sac causing swelling and inflammation. When a bursa becomes inflamed, moving your wrist becomes very painful and movement can be difficult. Any actions that put pressure on the inflamed bursa in your wrist can increase irritation and cause further inflammation and pain.

Wrist Bursitis Symptoms

If you have wrist bursitis you may be experiencing:

  • Pain in the wrist especially if the wrist is bent back and weight is put on it
  • A small lump or swelling at the top or side of the wrist

What Causes Wrist Bursitis?

Repetitive wrist movements (Repetitive Stress Injury) during work or activities that you do frequently (i.e. mouse movements, typing, sewing) is also a major cause of wrist bursitis. Sports with throwing or wrist twisting movements, such as baseball, badminton, and tennis can lead to wrist bursitis.

There are 2 different kinds of bursitis - "septic" and "aseptic" bursitis.

"Septic Bursitis" describes a bursa that is infected. Septic bursitis is not something to take lightly, as an infected bursa can spread infection throughout the body and become life-threatening. Consult your physician if you think you have septic bursitis as only they can confirm the condition and prescribe proper treatment options.

If the bursa is septic, getting to a doctor quickly may save your life!

"Aseptic bursitis" will happen when your bursa isn't infected but your bursitis has been caused by inflammation from another soft tissue injury. For elbow bursitis this might mean that you've hurt some of the tissue around your elbow from a trauma (like a fall) or from repetitive damage over time.

Visiting your doctor when you have wrist pain is always recommended, as there are many possible issues that can happen within the wrist and arm. Sometimes, one set of symptoms can result in multiple diagnoses. Some conditions exhibiting symptoms similar to wrist bursitis are as follows:

Carpal Tunnel Syndrome (CTS)

This is the common name for a group of problems that includes swelling, pain, tingling, and loss of strength in your wrist and hand. This condition will happen when the median nerve, which runs from the forearm into the hand through the carpal tunnel, becomes squeezed (compressed) by damaged tissue inside or outside of the carpal tunnel.

Flexor Tendonitis

There are two sets of tendons, called flexor tendons, that are responsible for fine movements of the hand. When flexor tendons contract, the hand and/or fingers will contract. They are located on the palm side of the wrist, and are made up of tissue called collagen and elastin. When these tendons become irritated, wrist and hand pain will manifest itself during movement. There will also be significant tenderness when direct pressure is applied to the area (wrist, palm side). If these tendons become strained, inflammation results and commonly the tendon sheaths become inflamed (tenosynovitis). Extensor Tendonitis

Extensor tendons are responsible for the extension of the fingers and hand. These tendons pass through the back of the wrist and, like the flexor tendons in the wrist, they are constantly active during hand and finger movements. If these tendons become strained, inflammation results and commonly the tendon sheaths become inflamed (tenosynovitis). Strains on these tendons can occur in many ways - (1) repeated forceful wrist flexion from use of a shovel or axe (2) repeated and excessive typing on a keyboard (3) harvesting produce in a way that requires constant and repeated extension/flexion of the hand (picking apples, corn tobacco leaves, etc). Much like the the flexor tendons, if these tendons become strained, inflammation results and commonly the tendon sheaths become inflamed (tenosynovitis). You may find the back of your hand and wrist quite swollen if you get this; in such cases, you will find that your ability to use your hand(s) will be severely limited until the swelling drops.

Ganglion Cyst

These show up as a mass or lump in the hand; most cysts are not cancerous and, in most cases, are harmless. However, it is important to note that nearly all cancers are capable of producing cysts.** They occur in various locations, but most frequently develop on the back of the wrist. Repeatedly apply stress to the wrist can cause a ganglion cyst. The cyst rises will out of a joint, like a balloon on a stalk. It grows out of the tissues surrounding a joint, such as ligaments, tendon sheaths, and joint linings. Inside the balloon is a thick, slippery fluid, similar to the fluid that lubricates your joints. Although many ganglions produce no other symptoms, if a cyst puts pressure on the nerves that pass through the joint, it can cause pain, tingling, and muscle weakness.

How Do I Diagnose Wrist Bursitis?

When you are experiencing wrist pain, visiting your doctor is always recommended, as there are many conditions that can cause wrist pain such as carpal tunnel syndrome, a fracture, a sprain, or bursitis.

To begin with, your doctor will gather a medical history about you and your current condition and symptoms. He/she will inquire about the intensity of your wrist pain, the duration of your symptoms and the limitations you are experiencing. Details about what what may have started the pain, when it started, and whether or not you have ever had treatments for this or a similar condition in the past, are very helpful in assessing your injury.

A physical examination will be performed to determine if you have any signs of wrist bursitis or other wrist injury. He/she will visually assess and palpate (feel) the bones and soft tissue in both your wrists to evaluate symmetry and recognize any differences. This will identify any abnormalities, such as swelling, bone deformities, atrophied muscles, redness and/or warmth on the skin. In many cases, the first sign that you have bursitis is swelling in the wrist.

Sometimes, one set of symptoms can result in multiple diagnoses. An x-ray or MRI is often needed in order to diagnose if the wrist is out of alignment or the extent of the soft tissue damage. Although rare, bursitis can be caused by an infection which can become potentially serious. Your doctor will be able to rule this out as a possible cause.

Fluid Removal - Your doctor may aspirate (use a needle) to remove fluid from the bursa to check for possible staphylococcus epidermis (or staphylococcus aureus) bacterial infection. The knee and elbow tend to be the joints most prone to septic (infectious) bursitis. If infection is detected you will be prescribed an antibiotic to treat it.

X-rays - X-rays will provide a two-dimensional image of the overall structure of your hand and wrist. Although bursitis cannot be diagnosed from an X-ray, they are helpful in identifying other possible causes of pain and/or ruling out other conditions. Some other possible causes of pain that an x-ray can identify include instability, bone spurs, abnormal bone shapes, wear and tear on the joints, avulsion fractures (when a fragment of bone tears away from the main bone along with the muscle, tendon, or ligament as a result of trauma), dislocations, arthritis, calcium deposits in the bursa or joint, and/or other problems.

MRI and CT Scans - You doctor may also order an MRI (magnetic resonance imaging) or a CT (computed tomography) scan for more detailed information to help evaluate the soft tissues (bursa, muscles, tendons, ligaments, fascia, and other connective tissues) in and around the wrist and hand. They will show fluid next to the muscle, and may identify bursitis, tendinitis, inflammation, tears and other associated conditions.

Diagnostic Ultrasound - Ultrasonography can confirm diagnosis and show tissue tears or thickened tendons. It is good for identifying bursitis and excessive fluid in bursa, and/or ruling out other causes of pain.

Bursitis Can Also Lead to Other Conditions and Injuries

Every time we use our joint where there is painful bursitis, tendons and muscle tissue move; when they disturb an inflamed bursa, every movement hurts. With injured tissue, pain happens - sometimes a LOT of pain, so we try not to move it. So, when doing something we need to use our affected joint for (climbing stairs, getting up from the sofa, moving a car wheel, walking to the mailbox), we start to use the non-affected joints more. In the case of shoulder bursitis, we use our other shoulder instead even though it is inconvenient. In cases of hip bursitis, you may find you are using your knees and back more - and so forth. Our body will automatically try to use other muscles to reduce pain even though it is inconvenient and you may not be fully aware it is happening. This concept of using other muscles/joints to complete a task is known as overcompensation.

A couple days later (after purposefully avoiding movement of affected joint), the pain has gone down (as well as the swelling) and this is where the trouble begins! The pain disappeared with the swelling but we still aren't fully healed.. Not knowing this, we start using our joint again and then the injury swells up again like it did before - all because the injury wasn't fully healed in the first place.

Eventually, we use the other leg/shoulder/arm more and more, in an attempt to lessen the pain. The problem is now that many of the muscles and joints you are relying on due to overcompensation are now being overused. This means that aches and pain now become commonplace in areas that you weren't expecting - all as a result of the original bursitis injury and the body's instinctive nature to "protect" it.

Everything in the human body is connected. Any soft tissue injury can lead to other injuries over time if not treated properly. You might start using your opposite arm or leg to perform normal daily activities, like carrying grocery bags or moving things around your house. You may also try to use the opposite side of your body to get back to work sooner to avoid waiting for your injury to heal completely.


Subacromial Bursitis Pain Pattern

Many of our clients have experienced pain in other areas of their body because they shift the "work" to the opposite side of their body. For example, a serious muscle strain in the right forearm can often lead to over-straining of the left forearm (a roofer would switch from holding a nailgun in the dominant, injured arm to the other weaker arm and likely strain that forearm quite easily).

We continually re-injure ourselves through our daily activities and now we are dealing with long term chronic injuries due to atrophy and overcompensation.

Muscle imbalances result, placing tension on bone and softer tissues - leading to a misalignment within the body. The extra stress on the opposite side of the body will also result in weakness and fatigue of soft tissue - increasing the chances they can eventually be injured or gradually degenerate. This is a textbook example of how you get an overcompensation injury. In some cases, physical therapists (PTs) warn that overcompensation can be very dangerous as it will affect gait or alignment on a permanent basis.


Trochanteric Bursitis Pain Pattern

An injury to one bursa can easily lead to straining in other areas, making an overall recovery hard to initiate. For example, a serious case of bursitis in the right knee can often lead to over-straining of the left knee and even hip. Most commonly, the injury occurs on the dominant side, so the risk of straining the other (weaker) side increases. The longer the injury (and corresponding pain) persists, the greater the chance that you will sustain more strain from overcompensation, eventually leading to issues in those areas as well. To minimize potential secondary injuries, know that it is important to deal with your bursitis injury quickly and completely.

Ignoring overcompensation pain and the pain in your original injury while returning to regular activities or your job can lead to even more problems with healing - like a chronic tendonitis/tendinosis injury or the formation of massive amounts of scar tissue in the area. Bursitis can turn into a vicious cycle - which is why it is absolutely critical to focus on methods that improve and strengthen injured tissue in a timely manner. The faster you truly heal, the less chance you have of sinking into a downward spiral of re-injury, scar tissue growth and overcompensation injuries.

Wrist Bursitis Home Remedies

In the following section, we outline effective conservative treatments for Bursitis in the Wrist and Hand.

First you need to stop the activity that is causing your pain and rest your hand. Focus on treating the symptoms of your bursitis and any other soft tissue injuries you may have in the wrist & hand. Make sure to significantly reduce inflammation, tenderness and swelling-induced pain first before switching to a goal of healing your bursitis injury.

When suffering from a soft tissue injury, your main goal should be to heal it quickly and completely while minimizing the chance of worsening your injury, or re-injuring it once it has started to get better. Obviously, a worsening injury will delay the healing process, but what's worse is that every re-injury and additional healing cycle increases the amount of time that your joint is immobilized. Time is not kind when it comes to an immobilized joint - you lose strength in the joint but worse than that, you lose the range of motion in your joint via atrophy (your flexible tendons, muscles and ligaments slowly shrink, decreasing joint elasticity). The more time that goes by with an immobile wrist & hand, the more likely you will wind up with a chronic joint problem (long term injury with persistent flare-ups). Basically it means that your wrist will not perform as well as it once did and it becomes more prone to injury again later on.

For non-infectious bursitis, the treatment starts with non-operative options such as RICE (Rest, Ice, Compression, Elevation) has been recommended for years by doctors, trainers, and other medical professionals to treat the pain and swelling, chronic pain from injury and after any re-injury. Bursitis injuries need rest and time for the tissue to heal. If your bursitis is caused by an infection (septic bursitis), the doctor will probably drain the bursa sac with a needle and prescribe antibiotics to treat the infection. Note that a septic bursa can quickly become life threatening - this is a major reason why you should get to a doctor for diagnosis asap if you haven't already done so.

The good news is that most cases of hand/wrist bursitis will heal with simple home conservative treatments and surgery is often not necessary.

Surgeons know that surgery will introduce more scar tissue into the joint - it is a known fact. This added scar tissue will be problematic, requiring visits to your physical therapist (PT) and conservative treatment options post-surgery. When it comes to bursitis there are only a few surgical options for treatment - injection, fluid drainage, or removal of the bursa sac and scraping of the bone(s) to allow room for the new bursa to grow back. This is why surgery is almost always only performed on chronic bursitis cases that have not healed despite a course of treatment using conservative treatment methods.

Conservative treatments for bursitis typically include:

  • Rest - This is important for initial recovery; rest and elevation will help reduce pain, swelling and inflammation in the early stages of injury. This can be difficult when you have to carry on with daily activities, but resting and elevating your wrist whenever you can is recommended. During your recovery you will probably have to modify or avoid the activities that put stress on the wrist until your pain and inflammation settle. However, too much rest can also be harmful to such injuries because immobility can eventually cause stiffening in the joints. This is why rest should be used when reducing initial pain and swelling, but should not be considered for more long-term conservative treatment.
  • Avoid Activities that Caused Your Bursitis - While resting your hand and wrist, it's also important to avoid all activities that may have caused the problem (especially any strenuous or repetitive wrist/hand/forearm movement). Continuing on with regular activities can increase the severity of an injury, turning a mild to moderate case of tissue damage into a downward spiral of worsening damage that may eventually severely impact your life. Also, trying to 'work around' your injury will eventually give rise to over-compensation injuries in other areas of your body.
  • Use a Cold Compress or Ice Pack for Inflammation - Cold is very effective at reducing pain and swelling - use at the onset of the injury and during flareups. Immediate pain relief and reduced inflammation can also relieve some of the pressure that's being placed on your bursa although cold temporarily shortens connective tissue (tendons, muscle, fascia) causing more friction on the bursa sac. Generally, use cold when there is swelling but stop once swelling is down. In cases of chronic (long term) bursitis, there is typically little to no inflammation.
  • Use a Wrist TShellz® Circulatory Boost Wrap - After inflammation and swelling have been reduced, you can use your own blood flow to maximize your rehabilitation. The intent of this is to provide you with increased range of motion, pain relief and strengthening of the surrounding tissue of the wrist. Doctors or surgeons typically won't perform a surgery until they feel that their patient has put effort into treating their injury with conservative treatment methods. This may include up to 4 to 6 months of visits to a PT clinic. If you haven't experienced any improvement in your condition during that time, surgery may be considered.
  • Stretching - Stretching your joint in PT and at home will help you to regain your range of motion much faster than not stretching at all. Stretching in many ways is key maintaining good Range of Motion (ROM) in your joint, and stretching can be made much easier with use of a Wrist TShellz Wrap® before to warm up tissue, and a Cold Compress or Ice Pack treatment after to prevent any return of swelling and inflammation.

Hand/Wrist Bursitis Recovery Stages


Hand/Wrist Bursitis Home Treatment Stage 1: If the wrist bursitis is very recent - use a good quality cold pack and Arnica Pain Relief Cream. If the injury has been going on for awhile and there is little to no swelling, skip to Stage 2 below.

Recommendation 1: If you are experiencing pain and swelling in the very early stages of the injury (first 48 to 72 hours), the first step in a conservative treatment protocol would be to focus on reducing the discomfort by applying a quality cold pack to the injured wrist along with a high quality pain relief cream such as our Arnica Infusion Cream.

Note: If you are suffering from a bursitis and/or tendinitis injury that has been going on for awhile, you may not need much cold treatment as inflammation may be minimal. Anytime you are experiencing significant pain, you may find our Arnica Infusion Pain Relief Cream to be quite helpful.

Not only will you use your cold pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...

  • after any activity that stresses the wrist in a moderate way. It is wise to quickly apply cold to not only reduce the pain, but to also minimize tissue damage that may have occurred during the prior event.
  • If you have a job that requires you to place some demands on your injured wrist. If that is the case, use cold immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
  • If you are in a post-operative situation where cold compression would be utilized heavily for at least the first few weeks following the procedure. Similarly, if you are scheduled for upcoming surgery, then your doctor or surgeon might recommend undertaking cold before the surgery.

Having a Cold Compress or Ice Pack available at home to use if and when the need arises is helpful. Many of you already do so.

However, too many people over-focus on cold while ignoring the most critical aspect to healing - nourishing the injured tissues with a healthy supply of blood flow needed to repair and rebuild the injured tissues.

Hand/Wrist Bursitis Home Treatment Stage 2: Focus on Increasing Blood Flow To Injured Soft Tissue - As This is The ONLY Way It Will Heal For the Long-Term

Even though the concept is simple, improving blood flow to injured soft tissue in the wrist can be difficult. Scar Tissue & Fibrotic Adhesions

All connective tissue is living tissue; tissue requires a blood supply and is continually in a state of regeneration. Any and all connective tissue has the potential for injury and inflammation. Injuries can be small such as in the case of a strain, or injuries can be large tears or even avulsions (complete tearing). Once an injury is experienced, the area will experience both inflammation and the growth of scar tissue (otherwise known as adhesions).

The occurrence of scar tissue is a regular part of life. Scar tissue forms as part of the progression after injury occurs. Scar tissue essentially 'fills in the gaps' after a cut, tear or wound presents itself in the body. Scar tissue is composed of the protein, collagen; it is widely known in the medical community that scar tissue is weaker and less flexible than original tissue.

  • Normal/original connective tissue is made up of collagen, but this collagen is aligned in a specific direction with each strand in alignment. This results in the connective tissue having a strong tensile strength while also having excellent flexibility and extensibility.
  • Scar tissue is made up of a 'criss cross/ random' formation of collagen and attaches to everything in the area. You can likely imagine the result of a patch of scar tissue; the effect is similar to pouring extremely sticky glue in your hair. It interrupts everything and is disruptive to the function of the connective tissue. The caveat is that the scar tissue does its job - it binds tears together. You definitely want the tear to heal, but it is important to ensure the scar tissue goes away despite the fact that it does not go away easily.
  • After scar tissue forms, although the wound or cut is now closed (and less prone to infection), the area of tissue itself is inferior in functionality to what it replaced.

Continuous re-injury and build-up of scar tissue makes it more likely that you will wind up with chronic pain, reduced Range of Motion or even arthritis (permanent damage).

Heal Quickly to Minimize Scar Tissue Growth & Reduce Re-Injury Risk

The human body will use scar tissue as a temporary solution and will try to build the scar tissue as fast as possible to heal tears in soft tissue (muscles, tendons). Scar tissue can form fast to bring together the edges of a tear, but working fast doesn't mean that the job's done right. When scar tissue forms it doesn't come together as neatly as regular (healthy) tendon/ligament tissue would. Scar tissue fibers will lay down over top of your tear in a cluttered, messy and jumbled up way.

This is how scar tissue works - it is messy and inelastic. The scar tissue that forms in your soft tissue will be unorganized and won't line up properly with the healthy tissue surrounding the tear. This scar tissue will attach to everything in and around the tear including the surrounding healthy tissue as well. This results in a long-term fusing together of everything the scar tissue touches, reducing your ability to move and making the injury even more painful!


For soft tissue injuries (tendons, muscles, ligaments, fascia), it is important to heal quickly and completely; this will help minimize the build up of scar tissue. If you don't, your tissue injury may be the start of a long term chronic injury resulting in a loss of Range of Motion and joint atrophy.

The longer a soft tissue injury remains, the more scar tissue is likely to build up in and around the injury location. This is why it is critical to treat your injury quickly without delay. Continuous re-injury and build-up of scar tissue while staying active or going back to work means you will experience a greater risk of winding up with on-going, chronic foot pain and overcompensation injuries to the hips, knee(s), and or back.

Surgery, Post-Surgery Rehabilitation and Hand/Wrist Exercises

Increasing flexibility and strength in the wrist and hand is a key component during recovery to maximize your healing and reduce odds of reinjury. Even when you're injured and in pain you need to keep moving to break-up scar tissue that's forming in tendons and ligaments from the forearm to the fingertips. Regular movement can increase healthy range of motion (ROM) of your joint, guaranteeing you a speedier recovery process and return to regular activities.

However, moving when you're injured is hard. Since moving while injured can be painful, most people think it's better to stop moving, rest their hand & wrist, then hope their injury will heal all on its' own. Even though rest is important to recovery, too much rest during the recovery process will increase the amount of scar tissue in the joint. Circulatory Boost treatments combined with a proper stretching regimen will help decrease proliferation of scar tissue while increasing elasticity and strength in the wrist and surrounding area.

During the healing process your body will fill in soft tissue tears with dense, brittle tissue called "scar tissue" (also known as "fibrotic adhesions"). The human body will use scar tissue as a temporary solution and will try to build the scar tissue as fast as possible to heal tears in the soft tissue. When scar tissue forms it doesn't come together as neatly as regular (healthy) tendon/ligament tissue would. Atrophy is arguably a bigger issue than scar tissue although they are closely related... Whether you have a muscle strain or have recently had an operation in the area, you will find that the joint has a more limited range of motion than you had before. In most cases, this is due to atrophy - the shortening of connective tissue surrounding the joint due to inactivity. In cases where atrophy exists, it becomes increasingly important to stretch and do gentle exercise to combat atrophy before it becomes a chronic (long-lasting) issue. If you are currently recovering from a hand or wrist injury that you have kept immobile for a substantial time, speak with your physician about stretches and exercise.

When done properly under recommendation of a physician, stretches have a proven track record of assisting in the speedy recovery of many patients. This is without doubt, the final essential tool for complete recovery. Once a patient deals with the swelling and inflammation through use of a Cold Compress or Ice Pack, then begins the Circulatory Boost process by using home treatment with the Wrist TShellz® Circulatory Boost Wrap, while resting to keep stress off the injury. Once the injury has healed, it is time to take the third and final step to recovery by beginning a rehabilitation stretching regimen.

Once you are ready, PTs often prescribe gentle stretching of an injured joint, especially if your are recovering from a particularly invasive surgery (ie reattachment, graft). Generally, this starts once the skin wounds have healed from the surgery (2 to 6 weeks). They don't prescribe this to annoy you - it is in fact the most important part of injury recovery. In some cases, the joint may never fully recover.

Consistent exercise & conservative treatment on a daily basis during your rehab while working with your doctor (or PT) is key.

Consistent stretching (as prescribed by your physician or PT) not only helps minimize the growth of scar tissue, but it also increases the elasticity and strength of the hand/wrist joints and surrounding connective tissue. Stretching, when used in conjunction with the TShellz® Circulatory Boost Wrap will allow you to be better prepared to handle higher and higher loads. Eventually you will reach a stage where your recovering wrist/hand can handle loads for longer and longer periods of time. This is the final stage of recovery - depending on age, the nature of the injury, your weight and many other factors, it can take the longest time to complete.
