Once your medical professional has diagnosed your condition, you can begin the most effective treatment that is appropriate for your bursitis. There are a variety of options available depending on the severity of your injury.
Important: An Infected Bursa Can Be Life Threatening
Before we go further, it is important to understand the severity and danger that an infected bursa can entail. If your bursa becomes infected it is referred to as septic bursitis. Septic bursitis is usually caused by a bacteria know as staphylococcus epidermis (or staphylococcus aureus) that is commonly found on the surface of the skin. Bursae that are located just below the skin are more susceptible to infectious bursitis due to abrasions on the skin covering the bursa. With infectious bursitis the bursa fills with pus instead of blood or fluid and the area surrounding the bursa appears read and is very tender.
In the case of septic bursitis, your doctor may prescribe antibiotics to fight the bacteria. Alternatively, the bursa may be drained every 3-5 days until the infection is resolved. Septic bursitis occurs more frequently in men than women, with 85% of the cases appearing in the male population. People who have diabetes, alcoholism, certain kidney conditions, or are undergoing steroid treatments are at higher risk of contracting septic bursitis. As with any infection, it is very important to see your doctor to receive treatment for septic bursitis. If left untreated, the infection may spread through the blood to other parts of the body and it can become life-threatening.
Okay, So I Have a Bursitis... What Should I Do Now?
If you think you have bursitis, make sure you have a diagnosis. If not, a visit to the doctor is always recommended as there are many conditions that can cause this type of pain such as a muscle strain, tendinitis, or even a fracture. If you do have bursitis, only a physician can rule out whether the bursitis is septic - know that septic bursitis is dangerous and can quickly become life threatening. Getting a proper diagnosis is important so you can treat your condition correctly. Once you have been diagnosed and your physician is not advising surgery, he/she will most likely recommend a conservative treatment protocol.
Bursitis Can Lead to Other Conditions and Injuries.
Every time we use our joint that has bursitis, damaged and swollen tendons and muscle tissue move - rubbing against the inflamed bursa. This is why every movement hurts. With injured tissue, pain happens - sometimes a LOT of pain, so we try not to move it. So, when doing something we purposefully avoid using the joint that has bursitis pain. If we can, we use our other joints instead even though it is inconvenient and difficult.
A couple days later the bursitis pain drops, and we start moving normally again - at this point the injury swells up again like it did before - all because the injury wasn't fully healed in the first place. We continually re-ignite the bursitis through our daily activities and now this injury has become chronic (long term bursitis).
Eventually, we use the other joints more and more. This is why the other joints start to hurt more - they are being overused now. Soon, aches and pain can become commonplace - all as a result of the original bursitis and the body's instinctive nature to "protect" the area - all because the injury wasn't fully healed in the first place!
Muscle imbalances result, placing tension on bone and softer tissues - leading to a misalignment in the body. The extra stress on the opposite side of the body will also result in weakness and fatigue of soft tissue - increasing the chances they can eventually be injured or gradually degenerate (ie. muscle strain and/or tendinitis). This is a textbook example of how you get an overcompensation injury. In some cases, physical therapists warn that overcompensation can be very dangerous as it will affect gait or alignment on a permanent basis.
An injury to one area can easily lead to straining in other areas - just due to the pain response in your biomechanics - making an overall recovery hard to initiate. For example, a serious muscle strain in the right hip can often lead to over-straining of the left hip. Most commonly, the injury occurs on the dominant side, so the risk of straining the other (weaker) side increases. The longer the injury (and corresponding pain) persists, the greater the chance that you will sustain more strain from overcompensation, eventually leading to issues in those areas as well. To minimize potential secondary injuries, know that it is important to deal with your injury quickly and completely.
Truly, living with bursitis can lead you into a vicious downward cycle - which is why it is absolutely critical to focus on methods that improve and strengthen the injured soft tissue in a timely manner. The faster you truly heal, the less chance you have of sinking into a downward spiral of re-injury, scar tissue growth and overcompensation injuries.
Non-Surgical Bursitis Treatments
If you suspect you may have bursitis, the first thing to do is consult your physician; only your physician can give you a proper diagnosis and from this, determine a course of proper treatment. After seeking medical advice, your physician will determine the degree of severity and whether or not it is infected (important!). An infected bursa is known as septic bursitis and can be life threatening - so get it checked out! In some cases of bursitis, your physician may recommend removal (known as a bursectomy) in which case a conservative treatment protocol would be recommended after your skin heals up from surgery.
In many cases, your physician will recommend conservative treatment options. A general outline of our conservative home treatment recommendations follow..
Stage 1: If the bursitis was incurred recently or if there is still significant inflammation and pain - use a good quality cold pack and Arnica Pain Relief Cream. If the injury has been going on for awhile and there is no swelling, skip to Stage 2 below.
If you are experiencing pain and swelling in the very early stages of the injury (first 48 to 72 hours), the first step in a conservative treatment protocol would be to focus on reducing the discomfort by applying a Cold Compress or Ice Pack to the injured bursa along with a high quality pain relief cream such as our Arnica Infusion Cream.
Not only will you use your Cold Compress or Ice Pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...
- after any activity that puts stress on your bursa (and surrounding soft tissue) in a moderate way. It is wise to quickly apply cold to not only reduce the pain, but to also minimize tissue damage that may have occurred during the prior event.
- If you have a job that requires you to place some demands on your injured joint. If that is the case, use cold immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
- If you are in a post-operative situation where cold compression would be utilized heavily for at least the first few weeks following the procedure.
- If you are scheduled for upcoming surgery, then you will need some type of cold temperature treatment to use so surgery can be initiated with minimal inflammation.
Having a Cold Compress or Ice Pack available at home to use if and when the need arises is helpful. Many of you already do so. However, too many people over-focus on cold while ignoring the most critical aspect to healing - nourishing the injured tissues with a healthy supply of blood flow needed to repair and rebuild the injured tissues.

Stage 2: Focus on Increasing Blood Flow To The Affected Bursa - As This is How the Body's Healing Process is Stimulated

Generally it is accepted that heat on soft tissue (muscles, tendons, ligaments) provides the following desirable therapeutic effects:
- Tissues heated to 45 degrees Celsius and then stretched exhibit a nonelastic residual elongation of about 0.5 to 0.9 percent that persists after the stretch is removed. This does not occur in these same tissues when stretched at normal tissue temperatures.

During the healing process and especially in surgery, scar tissue builds on injured soft tissue. Scar tissue is dense, non-flexible tissue that grows to fill in tears in tendons, muscles and ligaments.
